New Employee Information

Congratulations on your new postion! We’re glad you have chosen to join the company.

As a new employee, you will need to complete some paperwork prior to the first day of employment. For your convenience, the required forms are listed below and available for download. In addition, if you are a benefits eligible employee, you will need to familiarize yourself with our employee benefits programs and follow the steps to enroll in the plans.

New Hire Paperwork

The following forms must be fully completed, signed and accompanied by the required documentation, if applicable.

  • Employment Eligibility Verification (I-9 form): Under federal law, you must complete this form in person in order to verify your identity and eligibility to work in the United States. The required documentation is listed on the form.
  • Tax withholding form (W-4): These forms need to be signed. You must also supply your Social Security number.
  • Employee Information Form: Form to collect important information such as emergency contact numbers as well acknowledge receipt of employee handbook and other important documents.
  • Authorization for direct deposit: This form authorizes direct deposit of your paycheck. You will also need to supply a blank check or preprinted bank deposit slip that shows your bank account number.

Benefits Enrollment

If you are a new employee or newly eligible for benefits, you will need to log into MYPortal and enroll for benefits.

  1. As a full-time employee, you are automatically eligible for employer provided benefits including..
    • Basic Life Insurance (you will need to complete beneficiary information on the MYPortal site)
    • Long-Term Disability
    • Short-Term Disability

    These benefits become effective the first day of the month following 30 days of active full-time employment (or eligibility).

  2. You may enroll in the 401(k) retirement plan at any time. Your contributions will begin on the first of the month following enrollment. Matching contributions will begin after 90 days of continuous active full-time employment.
  3. You have 30 days from your hire date (or eligibility date) to enroll in…
    • Medical
    • Dental
    • Flexible Spending Accounts
    • Health Savings Account
    • Voluntary Benefits

    These benefit elections will become effective the first day of the month following 30 days of active full-time employment.

  4. If you do not enroll by your deadline, you will not have benefits coverage for the year unless you have a qualifying life event (marriage, birth of a child, etc.) and enroll within 31 days of that event. The next time you may enroll is during the next annual enrollment period.
  5. If you do not enroll in voluntary benefits when you first become eligible, you will need to provide Evidence of Insurability (EOI) to enroll in these benefits during the next annual enrollment.
  6. If you have a qualifying life event during the year, you may make limited benefit changes. To make a change, sign on to MYPortal within 31 days of the qualifying event to complete the proper documentation.

Here are the things you need to do:

  • Register as a new user on the MYPortal site and enroll in benefits within 30 days of your hire/eligibility date.
  • Designate a beneficiary for your life insurance. Your are automatically provided Basic Life and AD&D coverage, so be sure to designate a life insurance beneficiary even if you do not plan to enroll in any other benefits.
  • If you enroll in medical and dental coverage, make sure you receive your Insurance ID cards. You will receive two cards for you and your family members. If you do not receive your cards, contact the HR Department.
